Clinical and mathematical analysis of differences for various types of course in sporadic and familial forms of multiple sclerosis

Abstract

Background. This paper, based on the creation of a regi-ster of familial forms of multiple sclerosis (MS) on the model of the Kharkiv region, studies the features of different types of course (recurrent (RT), secondary-progredient (SPT), primary-progredient (PPT)) in sporadic and familial forms of the disease. Using the questionnaire method, we studied the clinical indicators that characterize different time stages in different types of MS: preclinical stage, debut, completeness, and duration of remissions after debut, a recurrent stage for RT and SPT, ways of forming the stage of progression for SPT and PPT, the stage of progression, including its variants such as steady, recurrent, progressive, and the rate of progression. Using clinical and mathematical analysis (permutation method), we obtained reliable clinical and diagnostic differences in the structure of clinical indicators at each stage of the course for sporadic and familial forms of MS. Materials and methods. One hundred and thirty-one patients were examined, including 87 (18 men and 69 women) with sporadic form and 44 (16 men and 28 women) with familial form. In the sporadic form, 55 patients had RT, 25 — SPT, 7 — PPT; in the familial form, 22 patients had RT, 15 — SPT, and 7 — PPT. Results. In RT, for the sporadic form, in contrast to the familial form, a favorable prognosis was more often met with a significant predominance of oligosyndromic debut and short relapses with rapid development at the relapsing stage. In the familial form, in contrast to sporadic, an uncertain prognosis prevailed, with more frequent (p < 0.05) prodromes of the debut, sphincter disorders in the debut, and long-term relapses with slow formation. In SPT for sporadic form, the uncertain character of the prognosis developed due to such clinical indicators as short debuts, long remissions following debut, recurrent variant of progression, and predominance of pyramidal and cerebellar symptoms at the stage of progression. In the familial form and an unfavorable prognosis, severe and prolonged debut, sphincter disorders and atypical syndromes in the debut, the formation of SPT immediately after the debut, bypassing the recurrent stage, prevailed. In PPT, the sporadic form with an uncertain prognosis was characterized by a long debut, the presence of a stabilization stage after the debut, and stem symptoms at the stage of progression. In the familial form, an unfavorable prognosis prevailed, which was characterized by an oligosyndromic and indefinite duration of the debut and the formation of a stage of progression immediately after the debut. Conclusions. The obtained patterns of clinical indicators, identified in different forms of MS, indicate a more unfavorable prognosis for all types of familial form compared to sporadic, primarily for progredient types of course.
