Cerebrocardial Risk Factors Under the Impact of Occupational Psychoemotional Stress

Abstract


Offered in 2013 by the European Society of Hypertension and the European Society of Cardiology, mechanism for assessing the 10-year risk of cardiovascular death from cerebrocardial events based on the level of arterial hypertension (AH) and the factors of the overall cardiovascular risk (FOCVR) made it possible to develop the strategy of therapeutic and preventive measures aimed at reducing the risks. Chronic psychemotional stress (CPES) is accompanied by an increase in overall and cerebrovascular mortality.
Objective of the study — to assess how much the impact of CPES triggers the formation of factors determining the prognosis for cerebrocardial risk and symptoms of focal brain lesions.
Study group (SG) consisted of 160 mainline locomotive drivers and assistant drivers (MLD and AD). By definition of International Labour Organization, this occupation is one of the most stressful. Comparison group (CG) was formed by 100 healthy men. Blood pressure control was performed by its 24-hour monitoring. Among FOCVR, we assessed smoking, overweight, dyslipidemia. Among asymptomatic damage of the target organs, we considered the intima-media complex (IMC) thickness of the common carotid artery and left ventricular mass (LVM).
In SG, the AH risk was 42.9 % (34.7 –51.2 %, р < 0.005), in CG — 2 % (0.2 –5.7 %). The risk for AH in MLD and AD was 21.4 times higher, than in CG (р < 0.001). The groups didn’t differ by number of smokers. Dyslipidemia was diagnosed in 24.8 % of MLD and AD and in 4 % in CG (2 = 19.55; р < 0.001). CPES is being often associated with overweight (2 = 14.56; p = 0.001), fat deposition around the waist (2 = 23.23; р < 0.001). In SG, IMC thickening (2 = 4.85; р = 0.027) and increased LVM (2 = 12.32; р < 0.001) are being detected more often. The 10-year risk of death from cardiocerebral events in MLD and AD was 68 %, in CG — only 8 % (2 = 32.55; р < 0.001).
CPES causes FOCVR formation and is associated with higher risk of cardiocerebral events, triggers the development of chronic cerebrovascular pathology.
