multiple sclerosis, types of clinical course, stages of recurrence and progression, nature of prognosis, gender differencesAbstract
There were examined the gender diversities in structural and functional organization of the recurrence stages (65 patients with relapsing course (RC) and 49 patients with secondary progressive course (SPC)) and stages of progression (70 patients with SPC and 60 patients with primary progressive course (PPC)) at different types of clinical course of multiple sclerosis (MS). Structural and functional characteristic of the recurrence stages consisted of analysis of such clinical parameters as severity and duration of certain relapses, the formation rates of clinical symptoms of relapse, completeness, duration and persistence of clinical remission between relapses; progression stages — forming path of progression in SPC and PPC, the rate and variants of progression, features of stabilization periods.
As a result of the conducted researches there were marked out the combinations of the clinical risk factors which have been selected according to the gender criteria which defined the character of the probabilistic prognosis at the subsequent stages of the disease. The proposed approach had clarified the role of gender factor, which greatly increases in terms of probabilistic prognosis in SPC compared with RC. After the transformation of RC into SPC the further course of MS in women in most cases is more is more severe than that of in men. The clinical stages of PPC had also occurred with the participation of gender factor, which manifested by more severe and unfavorable course of the disease in men.
