Differential Diagnostic Criteria of Initial Cerebrovascular Disorders in Patients with Metabolic Syndrome

Abstract


We have studied the features of cerebrovascular disorders in patients with metabolic syndrome (MS) compared with patients with no signs of MS. We have observed 80 patients with chronic cerebral ischemia and initial disorders of insufficient blood supply to the brain with symptoms of MS and without it. Patients underwent clinical and neurological examination, investigation of the cardiovascular system, duplex scanning of brachiocephalic vessels with color flow mapping. We have determined the parameters of blood, urine, blood glucose, hemocoagulation properties, atherogenic index, C-reactive protein, bilirubin, alanine aminotransferase, aspartate aminotransferase, total cholesterol, triglycerides, low- and high-density lipoproteins, glycated hemoglobin (HbA1c). Neuropsychological testing by Montreal Cognitive Assessment (МоСА) has been carried out. We have detected a violation of cognitive functions in patients with MS according to МоСА, which is most sensitive in mild and moderate cognitive impairment; it has been shown that in initial disorders of blood supply to the brain, there is no significant difference in the changes of cognitive functions between patients with MS signs and without them; in groups of patients with dyscirculatory encephalopathy of I degree and patients with MS signs and patients without MS, there is a difference in cognitive functions. We have found an inverse correlation between indicators of glycated hemoglobin and cognitive functions by МоСА (r = –0.44; p < 0.05).
